To begin the process of registering a new student, please complete the initial registration form by clicking the green button. Note that this is a lengthy form; you are able to save your progress at any time and return to complete it at a later time. Please contact the appropriate building registrar with any questions.

Required Registration Documents

Below is a comprehensive checklist for all new students, along with a few building-specific details.

1. Required Documents for All Students

Please provide the following documents for your child:

Proof of Birth: An original birth certificate with a raised seal, or a passport. Note: If you submit a digital copy, the original must be presented in person at a later date.
Proof of Residency:
- If you own: A copy of your mortgage or deed and copies of two different bills (utility, cable, moving, or mortgage) addressed to the property owner at the registration address. Cell phone or tax bills are not accepted as proof of residency.
- If you rent: A Landlord Affidavit, Tenant Affidavit, or Third-Party Affidavit, and copies of two different bills (utility, cable, moving, or mortgage) addressed to the property owner at the registration address. Cell phone or tax bills are not accepted as proof of residency.
Home Language Questionnaire: Available in English or Spanish.
Health Forms: A NYS School Health Examination Form, a Health History Form, and a current Immunization Record. Forms and more information can be found on the district Health Services webpage under "Required & Recommended Health Forms".

2. Additional Forms (As Applicable)

Depending on your student's specific situation, please also submit:

Transfer Students: A Records Release Form.
Special Education: A Special Education Release of Information.
Other Needs: A Free and Reduced Lunch application or a Foster Child Notification.

3. Building-Specific Instructions & Document Submission

Once you have gathered your documents, please submit them to your student's specific school building:

Bedford Road School (PreK-4): You may email your documents to brsregistration@pleasantvilleschools.org, or you can fax, mail, or drop them off at the BRS Main Office. Reach out to Laura Lalji at 914-741-1440 x40501 or laljil@pleasantvilleschools.org with questions.
- Incoming Kindergarten Families: Please also include a recent photo and a Dental Health Certificate (optional, but recommended).
Middle School (5-8): Please return all of your documents to the Middle School Main Office. Reach out to Catherine Giordano at 914-741-1450 x30511 or giordanoc@pleasantvilleschools.org with questions.
High School (9-12): Please return all of your documents to the High School Counseling Office. Reach out to Christine Purcell at 914-741-1420 x20601 or purcellc@pleasantvilleschools.org with questions.

Child Find

If you have a child between the ages of 3 and 21 that you have reason to suspect may be in need of special education services, you have the right to refer your child to the Committee on Preschool Education (CPSE; for children age 3 until age 5) or the Committee on Special Education (CSE; for children age 5 to age 21). For preschool-aged children, you may contact the District's CPSE Coordinator, Dr. Rachael Goldrich, and for school-aged children, you may contact the Director of Special Education, Dr. Rukmini Bhalla.
